Grant Request for Funding
Applications must be received by United Fund or postmarked on or before January 6, 2021, or delivered to the UW Extension office, 303 W. Chapel St., Dodgeville, WI by 3:00 PM that day.
The application asks for basic facts about your group, a specific description of your proposed project, program or event. This is in a PDF document for you to fill out. It also may be printed and completed as a handwritten document.
Our volunteer board cannot evaluate incomplete proposals. Consequently, before you submit your funding request, please be certain that you have provided all the requested information.

All applicants must provide:
Section A: If you received funds last year from United Fund of Iowa County attach a report summarizing the use of the funds and an accounting of how the funding dollars were spent. Describe the way(s) you publicized your United Fund award. Attach copies if possible.
Section B: The organization’s previous year’s financial report showing income and expenses. A copy of a Charter, Articles of Incorporation, IRS Letter of Determination – 501(c)(3), or Other Proof of non-profit status. Does your organization practice non-discrimination?
Section C: Describe your project, program or event. Please be as complete and thorough as possible.
